This condition often goes undiagnosed or is misdiagnosed as depression—because the hypomania may seem mild or even productive. However, the emotional rollercoaster can be distressing and disruptive.

  • Trouble concentrating or making decisions

Bipolar II is often confused with major depressive disorder (MDD), which can lead to ineffective treatment and increased risk of:

  • Emotional instability

Cyclothymic Disorder 

Cyclothymic Disorder (also called cyclothymia) is a chronic mood disorder involving frequent mood fluctuations—but less severe than those in Bipolar I or II Disorder.

Individuals with cyclothymia experience:

  • Periods of hypomanic symptoms (increased energy, activity, or irritability)

  • Periods of depressive symptoms (low mood, fatigue, pessimism)

However, these symptoms don’t meet the full criteria for hypomanic or major depressive episodes. The condition is often underdiagnosed or mistaken for personality traits, stress, or lifestyle instability.

⏳ Diagnostic Criteria

According to the DSM-5, to be diagnosed with Cyclothymic Disorder:

  • Symptoms must persist for at least 2 years (1 year for children/teens)

  • The individual must not be symptom-free for more than 2 consecutive months

  • The symptoms cause significant distress or impairment in social, work, or personal life

Hypomanic-Like Symptoms:
  • Elevated or irritable mood

  • Inflated self-esteem

  • Increased talkativeness

  • Reduced need for sleep

  • Racing thoughts or restlessness

  • Impulsivity or risk-taking

Depressive-Like Symptoms:
  • Low mood or sadness

  • Fatigue or low energy

  • Difficulty concentrating

  • Sleep and appetite changes

  • Feelings of hopelessness or low self-worth

Many people with cyclothymia struggle with emotional unpredictability, making relationships, work consistency, and self-confidence difficult to maintain.

Other Specified and Related Disorders

This diagnosis is used when a person has bipolar-like symptoms, but the pattern does not exactly match the criteria for a specific bipolar subtype. However, the clinician chooses to specify the reason.

Examples include:

  • Short-duration hypomanic episodes (e.g., 2–3 days instead of 4+)

  • Hypomania without major depression

  • Depressive episodes with some hypomanic features

  • Recurrent hypomanic episodes with minimal depressive symptoms

This diagnosis acknowledges that a mood disorder exists, and early intervention can be key to preventing progression or worsening symptoms.

🔸 Unspecified Bipolar and Related Disorder

This diagnosis is used when someone has symptoms consistent with a bipolar-related disorder, but:

  • The full criteria are not met

  • There’s not enough information to make a more specific diagnosis

  • Or the clinician chooses not to specify the presentation (e.g., in emergency settings)

This is often a starting point—used while symptoms are being monitored or during a first visit. It still allows for treatment and support to begin promptly.
